PCN Clinicians
Clinical Pharmacists
Ascent provide in-house Clinical Pharmacists to improve patients access to prescriptions, access to assessments for common conditions and improve medication safety. Each of the Network practices have their own Pharmacist to work in that surgery for their patients. The Pharmacists also have a responsibility to oversee the prescribing safety standards across the Network and work together and with the Multidisciplinary Teams to achieve this. All our Pharmacists are trained in, or training towards, additional qualifications in community care and prescribing, providing our patients with an enhanced access to high quality services. Our Pharmacy team are capable of advising on Blood Pressure and Cholesterol management, advising on alternative medications and over the counter options, streamlining your prescriptions and coordinating medication monitoring necessary for patients to safely continue certain medicines. They may be in touch proactively with our patients to arrange reviews and monitoring, or you may be offered an appointment with one via the Practice administration team, without needing to see a GP first.
Pharmacy Team
Ayesha Saiyed – supports Asplands Medical Centre, as well as being the Lead Pharmacist for the Network, assisting with training junior pharmacists and students. She has a wealth of experience in community pharmacy with extensive knowledge of over the counter treatments and minor illness assessments and is an independent prescriber. She will regularly provide detailed medication reviews, medicines optimisation and aids the surgery with prescription logistics. Ayesha works provides consultations via telephone and face to face from Asplands Medical Centre. Appointments can be booked via their Reception.
Sarah Baker – supports Fishermead Medical Centre providing regular medication reviews, prescription query responses, aiding with deprescribing and the processes of coming off certain medications when the time is right for you. Sarah is currently training to be an independent prescriber and is developing an interest in anticoagulation therapy (blood thinners) for certain conditions.
Izabela Kocan – came to us with experience in community pharmacies. She supports Walnut Tree Health Centre providing medication reviews, medicines optimisation and aiding with prescription logistics. She can help with “deprescribing” and the processes of coming off certain medications when the time is right. With her experience in the community pharmacy, Izabela has the advantage of being able to link with her local peers regarding any drug stock issues and available substitutions. She is training to become an independent prescriber.
General Practitioners
Dr Parisa Ghaffari (f) MD MRCGP (2023)- a qualified GP, who was trained at the Asplands Medical Centre and is well known to our senior clinicians and Woburn Sands patients. She works regularly for a practice in Buckingham during the week, but provides additional clinics for our PCN on alternate Saturdays, giving our patient's greater access to a reliable GP for routine concerns and long term condition management. Appointments for Saturdays at Fishermead or at Asplands (for Asplands and Walnut Tree patients) can be requested via your usual practice reception team.
Dr Neha Pruthi (f) MBBS MRCGP (2024) - a qualified GP, who was trained at the Asplands Medical Centre and is well known to our senior clinicians and the patient's of Woburn Sands. She works regularly for a practice in Buckinghamshire during the week, but provides additional clinics for our PCN on alternate Saturdays, giving our patient's great access to a reliable GP for routine concerns and long term condition management. She is also available at Asplands on a Tuesday for registered patients there. Appointments for Saturdays at Asplands (for Walnut Tree and Asplands patients) can be requested via your usual practice reception team.
