Role Overview - 

To act as a mental health resource to the PCNs by offering advice and support for the management and care of clients with mental health difficulties.
To provide/support monitoring of medication for all mental health patients, dementia patients and those suffering with anxiety and depression.
To support and develop self-management care plans for service users.
To utilise a range of therapeutic techniques

Monday to Friday 9am to 5pm

Role Overview - 

A Mental Health Social Prescribing Link Worker provides support for patients to look after their mental health; provide support for patients to join groups, find new opportunities, learn a new skill, meet people, address concerns with housing and finance in relation to mental health; assists patients to find activities in their local area that can help improve mental health and support patients to connect with their local community and other people to help manage any mental health issues.
