Change of Personal Details

If you change your name or address please notify the Practice by either:

• completing the online form
• requesting a Change of Personal Details Form from reception or
• by downloading the form below and sending it to the surgery or faxing it to Asplands on  01908 281597 or Woburn on  01525 290197.

There has been a change in Policy for the amending of patients Names and Dates of Birth by the NHS. If you need to do this we now require for name changes copies of Marriage Certificates, Deed poll documents.  If you have recently divorced and are returning to using your maiden name a copy of your Decree Absolute and a deed poll document showing that you are reverting to your maiden name.  In the case of amending 'Dates of Birth' a copy of your Birth Certificate would be required to support the change.
